Please note we are unable to book appointments without a referral, as we use a triage process to ensure you will be seen in an appropriate timeframe. Our reception team will contact you within 24 hours after receiving your referral with an appointment date and time. Please contact us if we have not reached out within two business days following a referral and our team will follow up accordingly.

Please note that the reception team is unable to organise an appointment for A/Prof Michael Hughes until he has personally triaged the referral.

 

New Patient Form

All new patients will need to complete our patient information form via email or SMS registration link. This will be sent to you after your appointment has been booked.

What to Bring

​

Please make sure you bring or send us all relevant information for your appointment, including:

•  Referral letter

•  GP Health Summary

•  All available test results

•  X-rays and scans

•  Recent reports from any other treating specialists if possible

•  Up-to-date medication list (prescribed and non-prescribed)

We highly recommend sending us as much information as possible prior to your appointment to ensure ample time to prepare your file.
