New Patient Form

All new patients will need to complete our patient information form via email or SMS registration link. This will be sent to you after your appointment has been booked.

What to Bring

Please make sure you bring or send us all relevant information for your appointment, including:

  •  Referral letter

  •  GP Health Summary

  •  All available test results

  •  X-rays and scans

  •  Recent reports from any other treating specialists if possible

  •  Up-to-date medication list (prescribed and non-prescribed)

We highly recommend sending us as much information as possible prior to your appointment to ensure ample time to prepare your file.

Fees

The Doctors at NSO are committed to providing the highest quality of care to all

patients. The length of a consultation will vary depending on the complexity of the presenting problem and each patient’s needs. This will determine the fee charged for the consultations. Learn more about our Fee Policy.

 

Cancellations

When patients do not show up or cancel without giving enough notice, they prevent other patients from being seen sooner. Cancellation made with less than 24 hours notice of your appointment will be subjected to a rebooking fee equivalent to your standard consultation fee. The payment will be credited to your upcoming consultation fee. This may be waived for extenuating circumstances at the discretion of the doctor. Late arrival to appointments may lead to rescheduling of the appointment.
